This is quite a unique, custom shotgun showing an extreme lateral offset; more than what would be typically expected for a simple crossover gun (for a a cross-eye dominant shooter (left-eye dominant but right-handed)). It is possible that this is a gun which was professionally customized for someone with mobility or physical limitations. The tang shows this to be a BSA shotgun, subsequently modified by “S.A. Guns, England” (marking on the rib between the chambers). Note: Markings on the barrel flats show barrels are chambered for 2.5″ shells and were sleeved (they were nitro-proofed).
